SS-31 (Elamipretide)
Scientific Overview, Mechanism, Clinical Use and Evidence

Scientific Overview

SS-31 (elamipretide) is a mitochondria-targeting tetrapeptide designed to improve mitochondrial function by interacting with cardiolipin in the inner mitochondrial membrane. It has been investigated across multiple mitochondrial and metabolic indications and, as of 2025, has received U.S. FDA accelerated approval for treatment of Barth syndrome under the brand Forzinity (elamipretide) injection.
Mechanism of Action

Elamipretide selectively associates with cardiolipin, a phospholipid enriched in the inner mitochondrial membrane:

  Stabilizes mitochondrial cristae architecture
  Improves electron transport chain efficiency
Reduces oxidative stress and reactive oxygen species
Increases ATP production and mitochondrial bioenergetics

Mechanistic work also supports a broader interaction landscape at the inner mitochondrial membrane consistent with modulation of mitochondrial function and resilience.
Biological and Clinical Effects

Preclinical studies demonstrate improvements in mitochondrial respiration, reduced reactive oxygen species, and functional benefits in models of mitochondrial dysfunction. In clinical development, elamipretide has been studied in primary mitochondrial myopathy and other mitochondrial disorders. FDA accelerated approval (2025) for Barth syndrome reflects regulatory recognition of clinical benefit in a rare mitochondrial disease context.
Time of Use / Treatment Duration

In published studies, treatment durations commonly range from 12 to 24 weeks for randomized phases, with extension studies lasting much longer (including multi-year open-label periods). Clinical benefit in certain endpoints may require continued therapy; discontinuation effects can vary by condition and endpoint.
Safety, Tolerability, and Precautions

Across clinical studies, elamipretide is commonly associated with injection-site reactions. Gastrointestinal symptoms and headache have also been reported. Use should be approached cautiously in complex multisystem disease, and monitoring should be tailored to the indication. Precautionary exclusions include pregnancy and lactation. For approved indications, contraindications and warnings should be followed per label.

Posology (Clinical Practice–Oriented)
Dosing should follow approved labeling when used for an approved indication.
Published clinical trial regimens commonly used elamipretide at 40 mg subcutaneously once daily (1 mL injection) for 12–24 weeks, with some protocols extending into long open-label phases.
For Barth syndrome (Forzinity), dosing should follow the official prescribing information and patient-specific factors (e.g., weight thresholds and monitoring requirements).
Use for other mitochondrial or metabolic goals should be conservative and physician-directed.
Clinical Practice Framing
From a scientific-medical perspective, SS-31/elamipretide is best framed as a mitochondria-directed therapy with a growing clinical evidence base in mitochondrial disease. Mitochondrial-targeted peptides are positioned as adjuncts to foundational interventions (nutrition, sleep, exercise tolerance, oxidative stress support) rather than stand-alone solutions.
